Auth failures MFA

Hoping someone can confirm something for me and possibly help with another. Logons failing at MFA stage. Looking at AzureAD logs I see a multifactor event (Authentication requirement = Multifactor authentication) with a status of “Success” but also a Failure reason of “Other.” It also has nothing in the “Multifactor authentication…
